Your physician or walk-in clinic provides a requisition form. For certain routine tests, no requisition is required.

Walk in or book ahead. Bring your requisition, health card, and any other ID. No referral needed for most tests.

Yes, most blood tests require a requisition from a licensed healthcare provider (physician, nurse practitioner, or specialist). Some routine tests — like basic wellness panels — are available without a requisition for a nominal fee.
Fasting requirements depend on the specific tests ordered. Lipid panels, glucose, and metabolic panels typically require 8–12 hours of fasting. Your requisition or physician will specify. Water and essential medications are generally permitted.
Most routine results are available within 24–48 hours. Specialty tests may take longer. Results are sent directly to your ordering physician and are also accessible via the patient portal.
Yes — walk-ins are welcome at all collection centres during operating hours. Booking ahead is optional but can reduce wait times at busier locations.
Bring your Ontario health card and photo ID. If billing private insurance, bring your insurance card and policy details. Your signed requisition is also required for most tests.
